Booking Process & Consent Form

Step 1: Pre-Screening & Eligibility Assessment

To ensure a safe and optimal experience, all participants must complete a pre-screening questionnaire before booking. This form will assess your important medical history, current medications, and psychological well-being to determine if you are eligible to participate.

  • The form must be completed honestly and accurately. Any false or omitted information may result in immediate cancellation without a refund.
  • If necessary, additional documentation (such as a medical clearance letter) may be required before approval.

Once approved, you will receive an email confirmation with instructions for the next steps.

Step 2: Payment & Reservation Confirmation

  • A non-refundable deposit is required to secure your spot.
  • Full payment must be completed at least before the ceremony.
  • Payment can be made via secure online platforms, bank transfer, or other authorized methods.
  • If you need to reschedule, please notify us at least one week before your booked date to explore alternative options.

Step 3: Digital Consent & Legal Agreement

Before finalizing your booking, you must digitally sign our Consent & Liability Waiver, which includes:

  • Acknowledgment of personal responsibility: You confirm that you are voluntarily choosing to participate, fully aware of the potential risks.
  • Medical disclaimer: You acknowledge that StarQuest is not a medical facility and that participation is at your own risk.
  • Code of Conduct agreement: You agree to respect the rules, facilitators, and other participants.
  • Legal compliance: You confirm that you are not carrying illegal substances, weapons, or engaging in any prohibited activities under Mexican law.
  • Confidentiality agreement: You agree to respect the privacy and sacred nature of the ceremony, including a no-recording policy to protect the integrity of the experience.

Once signed, a copy of your agreement will be sent to your email for your records.

Preparation Requirements

To ensure the best possible experience and safety for all participants, it is mandatory to prepare the body and mind accordingly:

  • Abstinence from alcohol and sexual activity: At least one week prior to the ceremony, participants must refrain completely from consuming alcoholic beverages and engaging in sexual activity, as these can affect the energy and physiological state necessary for the process.
  • No refunds after payment: Once payment has been made, full refunds are not available. However, if you are unable to attend your scheduled ceremony, you may reschedule for a later date. Your reserved spot will be reassigned to the next person on our waitlist.
  • Rescheduling and partial reimbursement: If you decide to cancel at least one week prior to your scheduled start date, partial reimbursements may be available for certain fees. However, 100% of the cost cannot be refunded due to the logistical and energetic preparations required for each ceremony.
  • Respect for sacred traditions: Our ceremonies are deeply sacred. We require all participants to honor and respect fellow attendees, the guide, elders, facilitators, and the sacred nature of each ritual.

Legal & Ethical Conduct

To maintain the integrity and safety of our ceremonial space, participants must adhere to the following ethical and legal standards. Failure to comply may result in immediate removal without a refund:

  • Respect for all individuals: Any form of verbal abuse, threats, or disrespect towards other participants, facilitators, elders, or hospitality staff will not be tolerated.
  • Prohibited substances: The use, possession, or distribution of any drug or substance illegal under Mexican law is strictly forbidden.
  • Weapons: No weapons of any kind are allowed.
  • Harassment and assault: Any form of sexual harassment, physical or sexual assault will lead to immediate removal and possible legal action.

Medical Conditions That Disqualify Participation

For your safety and that of the group, we cannot accept participants with the following conditions:

Cardiovascular Conditions

  • Heart disease, including tachycardia, arrhythmia, murmurs, pacemakers, recent cardiac surgery, or related complications.
  • Advanced hypertension (high blood pressure) or hypotension (low blood pressure).

Neurological & Psychological Conditions

  • History of schizophrenia, bipolar disorder, severe depression, psychosis, or other major psychiatric conditions.
  • Current or recent use of antipsychotic medication, nervous system inhibitors, or MAOI-based treatments.
  • Epilepsy, history of stroke, or any other neurological disorder.

Other Severe Health Conditions

  • Diabetes (advanced or uncontrolled).
  • Recent surgeries: Those who have undergone any major surgery within the past three months, especially related to the digestive system, cardiovascular system, or internal organs.
  • Liver disease and those currently under pharmaceutical treatment affecting liver function.
  • Cancer (advanced or undergoing active treatment).
  • Infectious or highly contagious diseases.
  • High-risk pregnancies.

Substance Dependency

  • Active addiction to cocaine, methamphetamine, synthetic drugs, or any illegal substances.
  • Dependence on prescription medications (including antidepressants, anxiolytics, opioids, or sleeping pills).
  • Use of birth control pills, hormonal treatments, industrial supplements, or any non-essential pharmaceutical medications during the retreat period.

Severe Allergic Reactions

  • Severe allergies to insect bites or other environmental factors that could be life-threatening in an isolated setting.

Final Considerations

StarQuest is not a detox program, a psychological therapy, or a medical treatment center. Our ceremonies are designed as spiritual experiences to receive guidance and wisdom from ancestral medicines, facilitated by experienced elders and guardians of tradition.

By registering for a ceremony, you acknowledge that you have read and understood these guidelines and that you assume full responsibility for your physical and mental well-being during the experience.

PARTICIPANT AGREEMENT & CONSENT FORM

Sacred Medicine Retreat and ceremony | StarQuest

1. INTRODUCTION

This agreement (“Agreement”) is a legally binding contract between [Participant’s Full Name] (“Participant”) and StarQuest (“Organization”). By electronically signing this document, the Participant confirms they have read, understood, and agreed to all terms herein before engaging in any ceremony or retreat involving Sacred Medicines.

2. GENERAL TERMS

1️⃣ Voluntary Participation – The Participant acknowledges that attending this retreat is their personal choice, free from coercion or obligation.
2️⃣ Non-Medical Program – StarQuest is not a medical or psychiatric institution and does not provide medical advice, treatment, or therapy.
3️⃣ Pre-Screening Requirement – Participation is subject to completing the Pre-Screening Questionnaire and being medically cleared.
4️⃣ Personal Responsibility – The Participant assumes full responsibility for their physical, mental, and emotional well-being during and after the retreat.

3. HEALTH & SAFETY CONDITIONS

📌 The Participant certifies that they DO NOT have:
✅ Severe heart conditions (e.g., tachycardia, pacemaker, recent surgery).
✅ Epilepsy, schizophrenia, bipolar disorder, or severe mental health disorders.
✅ Uncontrolled high/low blood pressure, stroke history, or high-risk pregnancy.
✅ Current addiction to synthetic drugs or substances prohibited by Mexican law.
✅ Recent major surgery (less than three months before the retreat).
✅ Infectious or highly contagious diseases.

📌 Medication Restrictions: The Participant agrees to discontinue (under medical supervision) any antidepressants (SSRIs, MAOIs, antipsychotics, etc.) at least four weeks before the retreat.

4. CODE OF CONDUCT

By signing this Agreement, the Participant commits to:
✅ Respecting all elders, guides, facilitators, and fellow participants.
✅ Refraining from alcohol, sexual activity, and drugs one week before and after the ceremony.
✅ Following all safety protocols outlined by StarQuest.

🚫 Grounds for Immediate Expulsion (No Refunds) 🚫
If the Participant engages in any of the following, they will be asked to leave the retreat immediately:
❌ Physical or verbal assault.
❌ Possession or use of illegal substances.
❌ Disruptive behavior affecting the group’s safety and experience.

5. LIABILITY WAIVER

The Participant acknowledges that engaging in Sacred Medicine ceremonies involves inherent risks, including but not limited to:

  • Psychological or emotional discomfort.
  • Unexpected physical reactions.
  • Intense spiritual experiences.

⚠️ Legal Disclaimer: By signing below, the Participant fully releases StarQuest, its facilitators, staff, and affiliates from any liability related to their participation in the retreat, including any physical, emotional, or psychological outcomes.
